How to make a referral

A referral can be made by parents or anyone involved in caring for a child or young person but parental consent is essential.

If you are aware of a child or young person who might benefit from our help, contact us to obtain a Referral Form.

The form must be completed in partnership with the child's parents or carers and information will need to be obtained from the paediatrician who knows the medical needs and prognosis.

Some parents may prefer to ask their paediatrician to complete the form.
